Host Homes

"Case management is not just a job; it's a calling to serve others with empathy, compassion, and unwavering dedication." - Unknown

Our host home providers offer a safe and nurturing environment where individuals in need of support and care can find a home and a caring community. We carefully match our clients with experienced providers who are trained to meet their specific needs. Our providers assist with daily living skills, such as hygiene, medication management, transportation, and other day-to-day tasks. They create a welcoming and inclusive atmosphere that prioritizes the well-being of our individuals and advocate for their needs, interests, and rights. Our providers actively encourage community engagement, accompanying individuals to access resources and social events. With their compassion, dependability, and commitment, our host home providers have a positive and transformative impact on the lives of those they support.

Our Host Home Case Management team prioritizes the happiness and safety of each individual, using a personalized approach tailored to their needs. We encourage our clients to set goals, achieve independence, and foster social connections within the community. Our case managers diligently monitor and support the safety and well-being of individuals, providing guidance, training, and ongoing support to both host home providers and individuals in their care. They collaborate closely with individuals and their teams to ensure the provision of high-quality care and ongoing support for providers. Our Case Management team is dedicated to promoting the overall growth and independence of our individuals, thereby improving their quality of life.

The Case Management Team

  • Teresa Chavez, Host Home Director

I have a Master’s Degree in Social Work and more than 30 yours of experience in the field of child and adult protection. I have been with Smith Agency for 4 years. I am proud to be a part of the Smith Agency and Host Home team. Our host home providers are the best around and they provide love and support, which makes me proud to work with them. I learn so much from the clients, case managers, and host providers every day and I could not ask for a better team. Thank you all for letting me be a part of this amazing program!

  • Toni Taylor, Associate Director Host Department

I have worked for over 20 years serving individuals and families in this field, this includes direct staff and managing the group homes before transitioning to the host home department. I also own a therapeutic massage business for individuals with intellectual disabilities. I enjoy meeting new people, spending time with family, and working in my yard.

  • Jessica Stoker, Host home case manager

I began working with this population in high school when I babysit kids with developmental disabilities. I have worked with Smith Agency for over 8 years, starting as a direct staff at the day program and moving up to Program manager, then transitioning to Host Home. I enjoy going on trips with my family, creating arts and crafts, and listening to True Crime podcasts.

  • Masey Burney, Host home case Manager

I started at Smith Agency day program after graduating High School in June of 2019 as a direct care staff and got my certificate in job coaching. I was then provided with the opportunity to work with the host home department. I am pursuing my degree in social work and plan to extend my studies towards my master’s degree once I complete my bachelors. When I’m not working, I enjoy traveling, spending time outdoors, and making people laugh.

Testimonials

“We have used Smith Agency’s services for many years to care for the needs of my developmentally disabled sister.  I have overwhelmingly found the staff to be kind, caring, and competent. They lead with compassionate hearts and take a team-oriented approach to managing the comprehensive care needed for my sister’s wellbeing and happiness. Smith Agency has done a great job over the years and continues to be a comfort to our family.”

While working with the host home department, they have helped support our journey in taking in an 18-year-old. The case manager has heard our specific needs and helped by giving ideas and talking to our client so that he is getting the message from other people, not just us.  This has helped by giving him multiple perspectives and guiding him to work on his independence.  They have also been flexible with scheduling as he has many appointments and needs that sometimes means last minute changes to home visits.”
